Applicant Identity Verification

Before a patient applies for a payment plan, identity verification is necessary to protect our providers and patients from fraud.

When possible, retain a copy of the applicant’s valid government-issued photo ID in their chart for audit and record-keeping purposes. Only one form of ID is necessary.

Acceptable ID Types:

  • Drivers License
  • State ID
  • Passport or Passport Card
  • Military ID/Dependent ID
  • Permanent Resident Card
  • Other Government Issued Photo ID

Spotting a Fake ID

There are a few ways the ensure the physical IDs being assessed are valid. We’ve summed up a few red flags to look out for.

    • Check for holographic overlays over the photo and/or laser engravings.
    • Check for misspellings.
    • Information listed in the incorrect location.
    • Rough or bumpy texture.
    • Verbal verification of patient information does not match information on the ID.
