Providers Report
This is a brief overview of the Provider’s reports, which allows corporations to see all Providers within their organization. This report is by request only and is delivered via email at the requester’s desired frequency.
A Provider is an individual practice that provides healthcare services to patients. A Parent Provider can have thousands of child Providers within its organization.

| Provider ID | The unique identifier for the Provider. | 12345 |
| Parent Provider ID | The unique identifier for the Parent Provider. Only populated if the Provider belongs to a Parent Provider organization. | 12345 |
| Parent Name | The name of the Parent Provider organization. Only populated if the Provider belongs to a Parent Provider. | Johns Parent Company |
| Legal Name | The legal business name of the Provider. | Johns Provider |
| DBA Name | The Doing Business As (DBA) name of the Provider — the public-facing name patients recognize. This may differ from the Provider's Legal Name. | Johns Dentistry |
| Company ID | A free-text field that Providers can use to assign a custom internal identifier to the Provider location (for example, a location code or branch number). | Location314 |
| Primary POC | The full name of the primary point of contact (POC) at the Provider location. | John Doe |
