Applicant Identity Verification

To help protect both patients and providers from fraud, it is important to verify a patient’s identity before they apply for a payment plan.

Ensure a clear and legible copy of the applicant’s valid government-issued photo ID is collected at the time of application and stored in the patient’s chart. This documentation should be retained for audit, compliance, and record-keeping purposes.

Acceptable ID Types:

✅    Drivers License

✅    State ID

✅    Passport (foreign or domestic)

✅    Passport Card (U.S. issued)

✅    Permanent Resident Card

✅    Other U.S. Government Issued IDs

 

‼️U.S. Military IDs are not accepted at this time.

Spotting a Fake ID

There are a few ways the ensure the physical IDs being assessed are valid. We’ve summed up a few red flags to look out for.

    • Check for holographic overlays over the photo and/or laser engravings.
    • Check for misspellings.
    • Information listed in the incorrect location.
    • Rough or bumpy texture.
    • Verbal verification of patient information does not match information on the ID.
